Abstract
A 27-year-old man suffered from bipolar disorder and lost consciousness after receiving an overdose of psychotropic medication. Three days later, at the initial visit, he was diagnosed with rhabdomyolysis and neuroleptic malignant syndrome, with a body temperature of 39.3 degrees, systemic muscle pain, red urine, and elevation of creatine kinase level. Tense blisters, erosions, and invasive erythema were scattered on the joints, forehead, and anterior chest of the extensor surfaces of the extremities. Pathologically, he was diagnosed with coma blister with subepidermal blisters, degeneration and necrosis of the eccrine sweat glands, and inflammatory cell infiltration of the superficial dermis. The rhabdomyolysis and neuroleptic malignant syndrome were treated successfully, and the erythema, blisters, and erosions were healed solely with topical treatment.
