We report the case of a man in his 50s who was attacked by a wild boar and suffered an open pneumothorax. The patient was gored in the chest by a wild boar while attempting to capture it in a trap. He also fell over and was bitten multiple times on both lower legs. Emergency services found an open pneumothorax ; therefore, the dispatch of a physician-staffed helicopter was requested. Prehospital chest tube drainage was performed, and the patient was taken to our hospital. The chest tube was removed on day 4 and the patient was discharged on day 28 without any complications. Injuries from boar attacks include both bites and deep puncture wounds and cuts from their large, sharp tusks that can be fatal. The possibility of life-threating chest trauma, vascular injuries, and multiple trauma should be carefully assessed when a patient presents with traumatic injuries caused by a boar.
