Background. Endobronchial lipomatous hamartoma is very rare. Cases. We describe the case of a 64-year-old woman who presented with persistent pneumonia. Computed tomography revealed a tumor obstructing her left lower bronchus. A lipomatous tumor measuring 2.2×1.7cm had invaded beyond the orifice of the lower bronchus. Bronchoscopic examination indicated the presence of a tumor at the orifice of the lower bronchus, and obstruction of the orifice of the upper bronchus was also identified. Although bronchoscopic biopsy did not yield a diagnosis of this tumor, a left lower sleeve lobectomy was performed because there was a possibility that the tumor was malignant. The pathological diagnosis was lipomatous hamartoma. The postoperative course was uneventful, and no findings of recurrence have been observed 3 years after surgery. Conclusion. Endobronchial lipomatous hamartoma is very rare. In the present case, the tumor was obstructing her left lower lobe and invading the outside of the left lower lobe bronchus.
