Abstract

Large randomized controlled trials (RCTs) are essential for generating reliable evidence in clinical medicine. Drawing on experience from international multicenter trials involving over 100,000 patients, this review examines why large RCTs are necessary and how they can be successfully conducted. Evidence from traumatic brain injury and bleeding studies highlights the limitations of small trials, systematic reviews, and observational research. The importance of unpredictability in allocation, simplicity in design, and large sample sizes is emphasized. Practical challenges, particularly recruitment and coordination across multiple centers, are discussed. The role of marketing strategies, including branding, engagement, and relationship-building, is explored as a critical component of success. Ultimately, large RCTs are a scientific and human enterprise requiring not only methodological rigor but also collaboration and trust.
