Basic and clinical studies on biapenem, a new carbapenem antibiotic, in respiratory tract infections

Abstract

We assessed the antimicrobial activity of biapenem (BIPM), a new carbapenem antibiotic, its serum and sputum levels and clinical efficacy in respiratory infections. The following results were obtained.
1) Antibacterial activity:
The minimum inhibitory concentrations (MICs) were measured for 576 strains of 17 species of clinical isolated organisms (158 strains of Gram-possitive cocci, 27 of Branhamella catarrhalis, 256 of family Enterobacteriaceae, 115 of Gram-negative rods and 20 of Helicobacter pylori) by the standard method defined by the Japanese Society of Chemotherapy, and compared with those of imipenem (IPM), carumonam (CRMN), ceftazidime (CAZ) and ampicillin (ABPC). Ampicillin was used for H. pylori. The antibacterial activity of BIPM was slightly less than that of IPM against Staphylococcus aureus (MSSA, MRSA), Enterococcus faecalis, and almost similar against Streptococcus pneumoniae, Branhamella catarrhalis. Against family Enterobacteriaceae and Gram-negative rods, BIPM showed the potent activity among the control drugs tested.
2) Sputum level
After 300mg of BIPM was administered to one of respiratory tract infection by intravenus drip infusion, sputum and serum level was measured. The maximum sputum and serum level were 1.38μg/ml at 1-2 h after administration and 35.4μg/ml immediately after administration, respectively. The rate of maximum sputum level to the maximum serum level was 3.9%.
3) Clinical results
BIPM was administered to 10 patients with respiratory infections at 300 to 1200mg per day b.i.d for 2 to 14 days.The clinical efficacy was good in 9 cases, unknown in a case because of adverse reaction which was observed in the asthmatic attack. Abnormal laboratory findings was observed in the elevation of GOT, GPT in 1 case.
