A 17-year-old female with severe cholinergic urticaria experienced multiple episodes of anaphylactic shock and showed a marked reduction in quality of life (QOL), despite treatment with antihistamines. She also had coexisting chronic spontaneous urticaria, dermographism, and heat urticaria. Plasma histamine levels were elevated even before provocation test, and symptoms were provoked by minimal stimulation during provocation testing. After initiating omalizumab for chronic urticaria, both chronic and cholinergic urticaria symptoms markedly improved, and no symptoms were provoked during subsequent provocation tests. This case suggests the potential efficacy of omalizumab for the treatment of cholinergic urticaria.
