Abstract
In the JSCCR Guidelines, surgical indications after endoscopic treatment are based on the analysis of submucosal invasive colorectal cancers that had undergone surgical resection, but the long-term prognosis remains unknown. In the current study, we evaluated the validity of the JSCCR Guidelines. One hundred and ninety-two pSM cancers, for which endoscopic treatment had been performed at Keiyukai Sapporo Hospital, were analyzed. According to the JSCCR Guidelines, we classified the patients into two groups, the curative and non-curative group. In the curative group, the recurrence rate was only 1.3% (local intra-mucosal reccurence). But, in non-curative group, the recurrence rate was 20.0% (local recurrence 10 cases, distant metastasis 4 cases). The mean recurrence period was 22.1 months. Our results suggested that the prognosis of the curative group was the same as for intramucosal cancer. If patients of the non-curative group did not undergo surgery, surveillance for 3-5 years was recommended. We should pay attention to submucosal recurrence during the follow-up period.
