Abstract
An 80-year-old man with a liver tumor underwent ERCP to examine the pancreatico-biliary system. Because of difficulty in selective cannulation for the bile duct, we tried to place a prophylactic pancreatic stent (5Fr, one flap) to prevent acute pancreatitis. During the procedure, the stent unexpectedly migrated into the main pancreatic duct (MPD). We avoided prompt removal of the migrated stent, because the edematous papilla might have presented problems. Alternatively, we additionally inserted a second stent (5Fr, bilateral flap) into the MPD to ensure the efflux of the pancreatic juice. On day 15 after the ERCP, we safely extracted the migrated stent as well as the additional stent. Temporary stenting over a migrated stent may be a safe and useful method to prevent pancreatitis until retrieval of the migrated pancreatic stent.
