A 68-year-old man presented to our hospital with a history of anal prolapse and bloody stool. The patient reported a habit of straining during defecation. CS revealed a rectal mass at the dentate line occupying three quarters of the circumference, suggesting rectal malignancy. However, histological evaluation of the biopsy specimen showed mild mucosal atypia with fibromuscular obliteration, leading to the diagnosis of mucosal prolapse syndrome. The patient was advised to avoid straining, and follow-up colonoscopies performed at three months, six months, and one year showed that the lesion had decreased in size. Here, we describe a case of mucosal prolapse syndrome mimicking rectal cancer that was treated conservatively.
