A 72-year-old man underwent screening EGD, which revealed a highly elevated lesion suspected to be a submucosal invasive carcinoma; however, narrow-band imaging (NBI) with magnification suggested that the lesion was confined to the mucosal epithelium and lamina propria. EUS identified the lesion as a mucosal epithelium/lamina propria carcinoma coexisting with a leiomyoma originating from the muscularis mucosae. Successful en bloc resection was achieved via ESD. Previous reports have highlighted cases in which submucosal tumors led to cancer depth overestimation, resulting in unnecessary surgical intervention. Preoperative evaluation, including EUS, is crucial to avoid overtreatment.
