Perforation is a serious complication associated with ERCP. It can occur in the gastrointestinal tract, papilla, bile duct, or pancreatic duct due to endoscopic manipulation or other procedures. Depending on the perforation site, life-threatening complications such as peritonitis, sepsis, and pancreatitis may develop. Although some cases can be managed conservatively, others require surgical intervention. Therefore, early diagnosis, appropriate treatment, and close collaboration with the surgical team are essential. Diagnosis relies on careful monitoring of the patient’s clinical condition and evaluation of endoscopic and fluoroscopic findings. A CT scan is recommended for confirming the diagnosis and assessing the severity. This article provides an overview of the causes and management of ERCP-related perforations, as well as basic preventive techniques and precautions.
