Abstract
We report a large cavernous hemangioma of the liver that exhibited extrahepatic growth. A 45-year-old male was admitted to our hospital on Nov. 1, 1986, because a hepatic tumor was detected during medical examination. An abdominal echogram showed a tumor protruding from the postero-inferior segment (S6) of the right hepatic lobe, which had hyperechoic and hypoechoic areas at the center and which appeared as a hyperechoic sphere. Abdominal CT scans also showed a low-density mass at the same position and it was enhanced as equally as liver parenchyma after contrast infusion. Angiography revealed some pooling from the arterial phase to venous phase at right-postero-inferior branches, but the caudally protruding area was avascula, r. In addition, laparoscopy also showed a spherical tumor at the lower edge of the right lobe ; its surface color was grey-white, and it was rich in vessels. Although most findings were compatible with a hemangioma, the risk of rupture was considered to be high in this case. Thus, the tumor was removed and segmentectomy of No. 6 was performed. The size of the tumor was 4.2 × 4.0 × 3.0 cm, and histological examination revealed it to be a cavernous hemangioma.
