Surgical site infection after laparoscopic hepatic resection

Abstract

Introduction: The number of laparoscopic hepatic resections (LH) has been increasing worldwide owing to better short-term outcomes and comparable oncological outcomes compared with those of open hepatic resection (OH). However, effect of LH for surgical site infection (SSI) remains controversial. Methods: Between 2007 and 2016, LH was performed in 193 patients and OH in 314, respectively. After propensity score matching, 117 patients in each group were selected. We investigated surgical outcomes including SSI between the two groups. Results: There was no difference in patient’s backgrounds, liver function, or tumor-or surgery-related factors between the two groups. Incidence of overall postoperative complications was lower in the LH group than in the OH group (20.5% vs 32.5%, P=0.038); however, there was no difference in incidence of overall SSI(2.6% vs 5.1%, P=0.499), wound infection (0.9% vs 3.4%, P=0.370) or abdominal infection(1.7% vs 1.7%, P=1.00)between the two groups. However, there was no patients with wound infection who underwent pure LH, which was tended to be lower than in the OH group (P=0.087). Conclusion: Pure LH may be effective for decrease in incidence of wound infection, compared with OH.
