Abstract
We report a case of bone marrow suppression that was suspected to have been caused by allopurinol. The patient suffered from chronic renal failure with conservative treatment, and had shown leukopenia, thrombocytopenia, and anemia even during allopurinol treatment. Thd blood concentrations of allopurinol and oxipurinol were not high, and LST to these two agents was negative. Various treatments led to incomplete remission thus preventing fatality. Although the hematological actions of allopurinol with resultant granulocytopenia and agranulocytosis have been reported, studies on bone marrow suppression are rare. We also report another patient with chronic renal failure in whom allopurinol induced pancytopenia. This patient had also shown leukopenia, thrombocytopenia and anemia, even during allopurinol treatment. The authors believe that allopurinol must be administered with caution to patients with chronic renal failure and reduction in blood cell counts.
