An autopsy case of 31 year old man with primary pulmonary leiomyosarcoma was reported with a discussion on the various nature of this tumor. The primary tumor (3.8×2.6×2.0 cm in diameter) arising from the peripheral region of the left lower lobe (S9), was well circumscribed, and yellowish-white, soft and friable. Microscopically, the tumor was composed of aggrigation of spindle shaped cells arranged in interlacing bundle. Fineststructure of the tumor displayed numerous microfibrils with dense patch in the cytoplasm. Partial resection of the left lower lobe was performed. However, the patient died 3 years and a half after operation due to the recurrence of the tumor. At autopsy, the recurrent sarcoma formed a huge tumor involving left thoracic cavity, subcutaneous tissue and upper abdominal cavity but no metastasis to other organs was observed. To our knowledge, present case is the 37th of primary leiomyosarcoma of the lung in Japanese literature.
