Abstract
A retrospective study of post-transfusion hepatitis (PTH) was done on 122 patients who underwent open heart surgery with blood component transfusion from September 1978 through March 1983. The criteria of PTH was defined as an elevation of SGPT over 200 units that occurred more than 3 weeks later after the transfusion.
Results were obtained as follows;
1. The incidence of PTH was 9% (11/122), in patients with the blood component transfusion, and ten of 11 patients with PTH were of type non-A, non-B. A half of 10 patients with type non-A, non-B were suffered from chronic hepatitis (SGPT > 100 units) more than 6 months after the blood transfusion.
2. The incidence of PTH was 20.6% (7/34) in patients who received over 5 units of platelet cell plasma, one of the blood component transfusion. On the other hand, the incidence of PTH was 4.5% (4/88), in patients receiving under 4 units of platelet cell plasma transfusion. The incidence of PTH in the former was significantly increased as compared with that in the latter (p<0.01).
3. The other blood component (fresh frozen plasma, concentrated red cell), a total amount of blood component transfusion and the other clinical factors relating to the inci-dence of PTH (age, cardiac lesions, bypass time, cardiopulmonary bypass time) were also studied, but all of these factors were not statistically significant.
These results suggest that the concentrated plasma (like platelet cell plasma) with high antigen value of hepatitis virus might increase the incidence of type non-A, non-B PTH subsequent to open heart surgery.
