A case of fusion of the third and fourth molars in the left mandible was presented. The patient was a 26 year-old female and her chief complaint of pain was in the third molar region. In the region, a large radiopaque mass was found and recorded on a pantomographic radiograph. In addition, histological findings showed that the mass was composed of the third and the fourth molars and their pulps were coupled each other. The fourth molar showed in this study, which is generally called the distomolar, is relatively rare.
