A pantomography of a cyst appearing in the right maxillary sinus of a 14-year old girl was susupected as being a dentigerous cyst. A marsupialization at the time of the biopsy was performed. Although the cyst was greatly reduced, it was thought that a tooth was contained in the cyst which was somewhat inferiorly located. A post-anterior X-ray finding a using contrasting medium taken at the time of the first examination was reviewed, and impaction of the third molar of the right maxilla was discovered. The tooth was touching the superior-external wall of the cyst. As a result of the pathological examination the cyst was removed, the impacted tooth was found in a superior-external direction, pushed out by the keratinized primordial cyst. It is to be emphasized that X-ray findings taken from several directions, using a contrasting medium are very important for a differential diagnosis of a dentigerous or primordial cyst appearing in the region of the maxillary sinus.
