Enema Reduction of Intussusception: 10 Years' Experience

Abstract
We encountered 325 cases of intussusception during the 10 years from 1995 to 2004, of which 47 cases had recurrences 1 to 7 times. We performed 398 enema reductions under fluoroscopic guidance for 405 intussusceptions, diluted barium method for 46 cases, diluted water-soluble agent for 183 cases, water soluble agent followed saline for 28 cases, and pneumatic reduction for 133 cases. The overall reduction rate was 88%, and there were no cases of intestinal perforation during this period. The reduction rate was 97% in cases seen less than 12 hours from onset, but 77% in cases seen after 13 hours from onset. The reduction rate improved as the reduction pressure became higher, and successful reduction was achieved in all the 27 cases in which higher pressure than 120 cm H2O was applied. Pneumatic reduction was superior to other methods: the reduction rate was 92% and the reduction time was 46 minutes. Six cases with failure of reduction were subjected again to enema reduction under general anesthesia before the operation, with successful reduction achieved in 4 of the cases.
