A Case of Ileocecal Actinomycosis in a Young Woman

Abstract

A 18-year-old woman visited our emergency room with right lower abdominal pain and fever. Her white blood cell count was 19,800/μL and the C-reactive protein level was 1.43mg/dL. Abdominal CT showed an abscess in the right lower abdomen. Our diagnosis was an abscess due to acute appendicitis. An emergency laparotomy showed edema and redness of the cecum and the ileum. Our diagnosis remained unchanged from before the operation, and we performed an ileocecal resection with abscess drainage. The resected specimen showed an abscess in the mesentery between the ileum and the appendix. There was no communication between the abscess and the cavity of the appendix. Pathological findings showed a colony of actinomyces in the abscess. There was no abnormal finding in the mucosa of the ileum and appendix. The patient was discharged in good health on the eighth postoperative day.
