Abstract
We analyzed the severity grade according to the guidelines for acute cholecystitis (AC), and compared the domestic guidelines with the Tokyo guidelines. Two hundred sixty AC patients who underwent cholecystectomy were enrolled. The AC severity grade according to the domestic guidelines was classified as Mild in 20.8%, Moderate in 45.4%, and Severe in 33.8%, while according to the Tokyo guidelines 43.1% were classified as Mild, 56.2% as Moderate, and 0.7% as Severe. In a comparison between all gradings of Severe, Moderate and Mild in both guidelines, there was a significant difference in the inflammatory findings. Furthermore, there was a significant difference in age, body temperature, and bactibilia. Therefore, large population-based studies related to the guidelines, especially the Tokyo guidelines, are required to ascertain the outcomes depending on the severity grade. Furthermore, revision of the Tokyo guidelines were considered necessary regarding the severity grade criteria depending on the results.
