Abstract
We report on a case of intussusception after an operation for ileus. A 78-year-old female was admitted to our hospital with lower abdominal pain, and X-ray imaging revealed ileus. She was managed with long intestinal tube decompression, but the ileus did not improve. Intraoperatively, we found that the small intestine was strangulated by the greater omentum adhering to scars from a past operation. We reduced the small intestine, and were able to preserve the small intestine as it was not necrotic. To decompress it we advanced the intestinal tube to before the strangulated part of the intestine. On day 3 post-surgery, the tube was removed. On day 7 post-surgery, the patient complained of nausea and began vomiting; enhanced abdominal computed tomography showed a target sign at the end of the duodenum. We then suspected an intussusception, and a second operation demonstrated an antegrade intussusception 5cm distal to the Treitz ligament which we repaired manually. The patient's postoperative course after the second operation was uneventful.
