Abstract
A 35-year-old woman visited our hospital complaining of abdominal pain. She had been suffered from constipation for 10 days. Abdominal CT revealed marked dilatation of the intestine which contained a large amount of feces. We diagnosed fecal ileus and began to treat conservatively. However, no improvement in her condition was seen. On laparotomy, severe dilatation of the colon was noted and a hard fecal mass was observed contained in the descending colon. We performed colon decompression and a transverse loop colostomy. Abdominal distention was prolonged after the operation and CT revealed massive ascites and free air on the 5th postoperative day. Therefore, we conducted an emergency laparotomy. We performed an extended right hemicolectomy and ileostomy because of the perforation of the transverse colon which caused panperitonitis with massive fecal ascites. It is very rare that a young healthy patient has been reported suffering from obstructive colitis caused by fecal ileus.
