A Case of Giant Meckel’s Diverticulitis with Abscess Treated by Laparoscopic Surgery

Abstract

The patient was a man in his 50s, who was referred to our hospital with the complaints of epigastric pain and vomiting. Abdominal CT showed a heterogeneous abscess cavity measuring 10 cm in size with a capsule in the left upper abdomen, penetrating the small intestine. Therefore, we performed emergency surgery under a tentative diagnosis of intraperitoneal abscess and localized peritonitis. Laparoscopic observation revealed a mass about 10 cm in diameter covered with a capsule on the opposite side of the ileal mesentery, about 60 cm from the ileocecal valve. We performed partial resection of the ileum including the mass. Postoperative histopathology showed that the luminal wall of the mass was a true diverticulum that retained the intestinal wall structure, and there was also evidence of diverticulitis due to acute inflammation, and the patient was diagnosed as having an abscess complicating Meckel’s diverticulitis. Repeated reversible stem torsion was suggested as the cause of the abscess formation. The postoperative course was good, and the patient was discharged on the 8th postoperative day. We report a rare case of abscess formation due to diverticulitis in a giant Meckel’s diverticulum, with a review of the literature.
