A 48-year-old man with abdominal pain was referred to our hospital by his previous doctor with suspected acute abdomen. Abdominal CT showed fluid collection and an enhancing intraabdominal tumor measuring 3.5 cm in diameter. Under a diagnosis of intraabdominal hemorrhage from the small intestinal tumor, we performed emergency surgery, and found bloody fluid in the abdominal cavity and a small intestinal tumor. The tumor was extraluminal, growing with a hematoma, so that we determined that the intraperitoneal bleeding was caused by rupture of the tumor. We conducted resection of the segment of the small intestine containing the tumor. Immunohistochemical analysis showed positive immunostaining of the tumor cells for c-kit and CD34. Therefore, we made a final diagnosis of GIST of the small intestine (high-risk group) . Three months after the surgery, the patient was initiated on adjuvant therapy with imatinib, and his condition remained satisfactory, without any metastasis or recurrence. Intraabdominal hemorrhage due to GIST of the small intestine is rare. We have reported a case of GIST of the small intestine with intraabdominal hemorrhage.
