Our hospital is a social medical corporation operated by four medical associations in Saitama City, and is a regional medical support hospital centered on referrals and emergency medical care. Since its opening, we have been attending to emergency patients 24 hours a day, 365 days a year, and the surgery department performs approximately 600 surgeries per year. In February 2021, the director called for an initiative for work style reform within the hospital, with the first goal being to ensure that clock-in and clock-out and overtime applications were carried out properly. In September 2022, a beacon-based clock-in and clock-out system was introduced to improve the efficiency of the clock-in management and overtime applications. The surgery department’s structure was changed from the attending physician system to a team system from fiscal year 2022, and a division of labor system was actively introduced. A junior resident and a full-time physician were paired up for emergency duty to facilitate the acceptance of emergency patients. Case reviews that were previously conducted outside of working hours began to be conducted during working hours, and the contents of the daily surgical conferences were shared thoroughly using cloud services. In order to obtain permission for overnight duty, we began to maintain detailed work diary records in January 2022, and the surgical department was granted permission for overnight duty in June. We report our efforts so far and the tasks ahead.
