Necrosis of the remnant stomach is a rare and serious complication of distal gastrectomy. In this report, we describe a case of necrosis of the remnant stomach after distal gastrectomy, which was successfully treated by urgent laparotomy. The patient was an 82-year-old man who had undergone distal gastrectomy for gastric cancer. On the 7th postoperative day, contrast-enhanced CT showed poor contrast in the gastric wall, and we diagnosed the condition as necrosis of the remnant stomach. Intraoperative exploration via an urgent laparotomy revealed that the entire gastric wall around the anastomosis was necrotic and partial gastrectomy was performed. Postoperatively, we observed duodenal suture failure, but the patient was discharged from the hospital after conservative treatment.
