An 83-year-old woman was admitted for abdominal distension, and abdominal contrast-enhanced CT scan revealed a left ovarian cyst and dilatation of the descending colon. Blood flow to the sigmoid colon and left ovary was maintained. Colonoscopy revealed no abnormalities on the mucosal surface of the sigmoid colon, however circumferential stenosis was observed, and we inserted a transanal ileus tube. After the intestinal dilatation improved, we performed surgery on the fourth hospital day. Intraoperatively, the left ovary was found to be twisted. An epiploic appendage of the sigmoid colon was caught in the torsion of the ovary and pulled, causing sigmoid colon obstruction. We separated the epiploic appendage and removed the left ovary. The patient was discharged on the 22nd postoperative day. We encountered a case of sigmoid colon obstruction associated with torsion of the pedicle of a left ovarian cyst, and we report this case with a review of the relevant literature.
