Abstract
We describe the surgical anatomy and several approaches to the lateral ventricle. This part is separated to anterior horn, body, trigone, posterior horn and inferior horn. Several parts of the lateral ventricle for surgical anatomy are shown, and anatomy of velum interpositum is also shown in this article. Main surgical approaches to the anterior horn and body are transcortical approach and frontal transcallosal approach. There are four main approaches for the atrium as follows; a high superior parietal approach through the parietal convexity, a lower posterior parietal approach through the inferior surface of the parietal lobe, an interhemisphric transcallosal or transcortical approach through the precuneus area, and a transsylvian approach. The high parietal lobe approach should be recommended in the dominant cerebral hemisphere for avoiding the high cognitive functions. And surgical approach for inferior horn is transsulcal or transgyrus approach. The knowledge of microsurgical anatomy and several surgical approaches for the lateral ventricle is necessary for safer and less invasive micro and/or neuroendoscopic surgery.
