Abstract

  STA-MCA bypass surgery is currently widely performed to prevent the recurrence of occlusive cerebrovascular disease. But historically there is a background of negative studies that have been published about the bypass surgery. First, there was the Japanese EC-IC bypass Trial (JET study) that proved the superiority of EC-IC bypass surgery. Then, the Carotid Occlusion Surgery Study (COSS) denied the effectiveness of bypass surgery again, but at the same time also advanced its verification. With such conflicting reports, it seems necessary to announce real-world results including perioperative complications, in order to elucidate the divergence of the results of JET and COSS. In addition, the future potential of STA-MCA bypass surgery is bright with the expectation of new developments such as acute phase bypass surgery and with the incorperation of regeneration therapy.
