The clinical evidences of endovascular therapy (EVT) for symptomatic intracranial artery stenosis (ICAS) were reviewed, and the questionnaire results to the members of the Japanese Society for Neuroendovascular Therapy were introduced. Two randomized controlled trials (RCTs) for symptomatic ICAS failed to show the efficacy of EVT. Therefore, aggressive medical treatment is now the primary option. A total of 783 EVTs in 261 institutes were reported to a nationwide questionnaire. The patients with acute occlusion or with symptomatic, medically refractory, and cerebral blood flow (CBF) reduction were frequently treated by EVT ; however, those asymptomatic lesions without CBF reduction were conservatively treated. New clinical trials are needed to determine the appropriate treatment for ICAS in the near future.
