The most important knowledge for endovascular surgery is the functional vascular anatomy associated with the vascularization of the cranial nerves and regional neural tissues. Functional vascular anatomy is the study of anatomy concerning the function that determines the normal and pathological vascularization of the brain and spinal cord. This requires a precise analysis of the regional and global circulation of the vascular system. Evaluating the collateral circulation under the balloon test occlusion is also a part of neurointervention. It is mandatory to understand and define the potential anastomosis between the internal and external carotid arteries angiographically. These functional aspects of the cerebrovascular system are reviewed in this article.
