Abstract

A 66-year-old man who had undergone sigmoidectomy for sigmoid colon cancer in 1985 has suffered from sporadic episodes of rectal bleeding on defecation since January 1990. A small elevated lesion with central depression was recognized at descending colon by barium enema study and colonoscopy. Under diagnosis of metachronous colon cancer, left hemicolectomy was performed. The tumor was round-shaped and showed steep elevation with deep central depression, measuring 1×1cm in diameter. The edge of elevation is partly covered with normal mucosa. Microscopic findings showed infiltrative growth of moderately differentiated adenocarcinoma towards the deep layer. Cancer cells invaded through the proper muscle layer infiltratively and reached the subserosal layer. No adenomatous component was noticed. We report this case because small advanced colon cancer less than 1cm in diameter invading the subserosal layer is quite rare.
