Abstract

 When the Patch Test Panel® (S) entered the Japanese market in 2015, the screening test for causative allergens in contact dermatitis became much easier. Now, many dermatologists use patch testing and demonstrate their highly skilled diagnostic ability based on the patch test's results. On the other hand, it is not easy to perform and judge the patch tests adequately. Expert knowledge and training are necessary. When using patch testing, several problems and questions occur. In this paper, I demonstrate the benefits of conducting patch tests using standard allergens when presented with cases of contact dermatitis. Furthermore, I comment on some questions relating to the troubles associated with patch testing, such as 1) the difficulty in distinguishing between allergic reactions and toxic reactions, 2) the difficulty in conducting a patch test because of persistent refractory eczema on the patient's back, and 3) the difficulties associated with getting patients to come to the clinic for the "day of judgment." When a dermatologist treats a patient with refractory eczema, it is important to investigate whether or not the patient is exhibiting allergic contact dermatitis and to confirm the diagnosis with a patch test.
