In bowel obstruction following laparoscopic sacrocolpopexy, strangulating intestinal obstruction is a serious complication that requires emergency surgical removal. Most instances are caused by an internal hernia penetrating the bowel in the gap produced by non-physiological tension when fixating mesh and in the event of retroperitoneal high position closure of the cul-de-sac, which can also be called iatrogenic complications. In this article, we will discuss two cases of postoperative strangulating intestinal obstruction and examine retroperitoneal sutures. We also introduce retroperitoneal covering sutures that utilize the right uterosacral ligament to reduce the likelihood of gap formation and alleviate the stress load on the mesh during promontory fixation.
