Abstract
Recently, risk management has been an urgent issues in hospitals in Japan. Incident reporting system was started in our hospital in order to prevent medocal accident since 2000. 1445 incident reports has been collected for only one year in 2001. There were much more incidents as to drug event, nursing care, fall down. It was effective that we has developed an incident report system as a part of medical risk management, not only to prevent medical accidents but also to make a quality and cost benefit.
