Study of Issues in Medication Management Performed by Family Caregivers Providing Home Care to Patients at the End of Life and Support Needed
Using Japanese and International Publications as a Clue

Abstract
This study aims to identify issues in medication management performed by family caregivers and the support they require, based on a literature review of domestic and international studies, and to provide insights into key strategies for ensuring safe and effective medication management for patients receiving end-of-life care at home. A literature review was conducted using PubMed and the Japan Medical Abstracts Society (Ichushi Web) to select original articles that addressed issues and support needs related to medication management by family caregivers for end-of-life patients receiving home care. Analysis of 23 articles (21 international and 2 Japanese) revealed several challenges in medication management by family caregivers, including “Lack of skills by family caregivers,” “Various obstacles,” “Impact of the patient’s mental and physical condition,” “Disagreements among family members,” “Inadequate professional support,” and “Heavy burden on family caregivers.” Furthermore, the support needs identified were “Skill development through instruction,” “Establishment of systems and guidelines,” “Optimal medication prescriptions for patients and caregivers,” “Facilitation of communication,” “Comprehensive support tailored to family caregivers,” “Provision of public support when necessary,” and “Adherence to a professional attitude.” To enable family caregivers to perform safe and effective medication management for end-of-life patients at home, adequate guidance from healthcare professionals is essential. The development of systems and guidelines to provide timely and appropriate support, as well as the prescription of optimal medications for both patients and family caregivers, is critical. Moreover, healthcare professionals should foster trust with family caregivers and maintain a compassionate approach that aligns with the life wishes of both the patient and their family.
