After chemoradiotherapy (CRT) for nasopharyngeal carcinoma, natural closure of tympanic membrane perforations becomes difficult due to Eustachian tube dysfunction, middle ear mucosal fibrosis, and local vascular impairment. We report a case of tympanoplasty using a postauricular periosteal-pericranial flap for tympanic membrane perforation following CRT, achieving favorable healing. The use of a well-vascularized flap suggests the possibility of successful tympanic membrane closure even in the poor healing environment after radiotherapy.
