Abstract
Acute mastoiditis is well known to be a common complication of acute otitis media in children. Due to recent advances in antibiotic treatment, the incidence of acute mastoiditis with a typical course has decreased, however, the occurrence of masked mastoiditis appears to be increasing. In adult cases, this disease is mostly secondary to chronic otitis media or cholesteatoma. In this paper, we report the case of a 33-year-old male who suffered from classical, acute mastoiditis with otologia and headache with a history of acute otitis media without any general diseases. He showede typical findings of acute mastoiditis, which consisted of swelling over the mastoid and swelling in the lateral external auditory canal wall. In the CT findings, the presence of cholesteatoma was suspected. We performed a mastoidectomy and found no cholesteatoma, but the cavity was full of debris. As a result, we performed a necrotomy. The patient showed a good recovery and was discharged on the 10th postoperative day. The present case was thus considered to demonstrate classical acute mastoiditis without cholesteatoma.
