We report two cases of spontaneous mediastinal emphysema. A 34-year-old male (case 1) and 16-year-old male (case 2) developed pain in their necks and chests after shouting (case 1) and playing football (case 2). Neck and chest CT revealed mediastinal emphysema. Both patients were treated with bed rest and antibiotic therapy, and recovered completely within three days.
