Four cases of hypopharyngeal cancer in 87-year-old males are reported. All cases originated from the piriform sinus and were treated with pharyngo-laryngectomy or pharyngo-laryngo-esophagectomy under general anesthesia (1 case) or local anesthesia (3cases). Three cases were in stage III and one case was in stage IV. We emphasized the precise examination of accompanying complications, the physical and psycho-social condition and the value of subsequent radical surgery for the purpose of evaluating quality of life of the aged patients.
