Acute acrylamide poisoning: a case report

Abstract
Acrylamide is used for various purposes, especially for industrial processes. The acrylamide monomer causes chronic poisoning and a peripheral neuropathy, but acute poisoning is rare. We report a case of acute acrylamide poisoning wherein a 23-year-old man attempted suicide by ingesting acrylamide solution and was brought to our emergency department. His general condition was good and his vital signs were stable. Physical and blood examinations yielded normal results. He became restless 9 hours after acrylamide ingestion; therefore, we administered a sedative and admitted him for observation. Subsequently, his entire body stiffened and started sweating. His blood pressure decreased and lactate level increased 11 hours after ingestion. Fluid resuscitation was not adequately effective, and catecholamine infusion and intubation were required. However, his circulation did not stabilize and he developed multiple organ failure. Computed tomography showed a small amount of ascites and diffuse edema of the intestinal wall. We thought that the patient had ischemia of the small bowel, but surgery was not possible because of unstable circulation. He died 40 hours after ingesting acrylamide. Since acrylamide poisoning symptoms do not appear immediately after ingestion, such patients should be closely observed, regardless of their good condition on arrival at the hospital.
