Surgical techniques for traumatic duodenal injuries in a tertiary emergency center

Abstract
Background: Duodenal injuries resulting from blunt trauma are relatively rare and present challenges in surgical intervention and perioperative management. This study aimed to retrospectively analyze surgical techniques and perioperative care for traumatic duodenal injuries. Methods: A retrospective review was conducted on cases of traumatic duodenal injuries treated at our hospital between January 1, 2002, and December 31, 2023. Results: Eight cases were analyzed, with a median age of 46 years and a male predominance (50%). Most injuries were classified as AAST Grade III (75%). The surgical procedures were simple suture closure of the duodenal injury in 4 cases (50%), simple suture closure and jejunal serosal patch in 2 cases (25%), and filling round ligament of liver to perforated duodenum and pancreaticoduodenectomy in 1 case each (12.5%). Clavien Dindo Grade 3a or higher complications occurred in 3 patients (37.5%), and the median postoperative hospital stay was 60 days, and all patients were discharged alive. Conclusion: Simple suture closure proved effective in managing traumatic duodenal injuries, underscoring its significance as a primary surgical approach in such cases.
