Abstract
We report a case of pyogenic liver abscess caused by advanced gastric cancer invading hepatic capsule directly, and operated for in emergency. A 58-year-old man complained of upper abdominal pain and slight fever in the previous hospital. His examinations revealed advanced gastric cancer with necrosis and infection. He was admitted and treated with antibiotics, but his general condition had getting worse. He moved in our hospital. The computed tomography showed liver abscess caused by direct invasion of gastric cancer to the liver. Total gastrectomy en bloc resection with left lateral segment of the liver was performed in emergency. Resected specimen included fistula between gastric cancer and liver. Histological examination revealed direct invasion of the liver capsule and no intrahepatic malignant lesion of the resected liver. It is rare case that liver abscess was formed by direct invasion of gastric cancer. We think it is important to remove stomach en bloc with the liver including liver abscess surgically in this case.
