A Case of NSAIDs-Induced Bleeding Small Intestinal Ulcer Detected by Intraoperative Endoscopy

Abstract
We report a case of non-steroidal anti-inflammatory drugs (NSAIDs)-induced bleeding small intestinal ulcer with massive bleeding. A 79-year-old woman was admitted to the hospital because of abdominal pain and melena. She had taken NSAIDs for chronic lumbago for a year. The source of bleeding could not be detected by upper gastrointestinal endoscopy, colonoscopy and abdominal angiography. Massive bleeding caused severe anemia (Hb3.6g/dl) after four days of hospitalization. Emergency laparotomy was performed for uncontrollable bleeding. Intraoperative endoscopy detected a bleeding ulcer in the ileum and partial resection of the ileum was conducted. Histological examination showed no specific lesion such as granuloma, vasculitis and malignancy. The definitive diagnosis was bleeding small intestinal ulcer caused by NSAIDs.
