Laparoscopic-Assisted Resection of Appendiceal Myxoglobulosis: A Case Report

Abstract
The case is a 65-year-old woman who has been visiting our hospital for treatment by chronic rheumatoid arthritis and myoma of the uterus. Appendiceal swelling was revealed on an abdominal CT scan conducted during periodic investigations. Exterior wall displacement suggestive of a submucosal tumor was revealed in cecum by colonoscopy. MRI examination found low signal intensity in the appendiceal lumen on T1-weighted images and high signal intensity on T2-weighted images. T2 fat suppression images showed a non-uniform interior, suggesting uneven liquid components. We performed a laparoscopic-assisted ileocecum resection by the diagnosis of appendiceal mucocele from the preoperative imaging. The resected section showed that the appendix was swollen and the appendicular root was protruding into the cecum. The appendix was filled with semi-transparent to yellow granular structures. Histopathological examination found that the appendiceal mucosal epithelium was displaced by mucus and was flattened, but no malignant findings were apparent. The patient was diagnosed with appendiceal myxoglobulosis along with an appendiceal mucocele. Appendiceal myxoglobulosis is an extremely rear case, and it is difficult to diagnose preoperatively. We herein report this case with a discussion of the pertinent literature.
