A woman in her 20s was transported to our hospital by ambulance with unconsciousness after an overdose of perampanel. Upon arriving at the hospital, she exhibited a disturbed state in which she was screaming and violently moving her extremities. After the administration of activated charcoal and laxatives, she was hospitalized for observation. On the 2nd day, she excreted stools containing the activated charcoal. On the 4th day, she regained the ability to converse with hospital staff. She displayed severe psychiatric symptoms of aggression until the 5th day but was discharged from the hospital on the 6th day once her symptoms improved. Blood levels of perampanel remained high during the hospitalization, possibly due to metabolic saturation or redistribution of the drug after the overdose. The symptom improvement may have been due to a decreased number of receptors or decreased drug affinity.
