Abstract
The Japan Dietetic Association dispatched volunteer members to the affected areas of the Great East Japan Earthquake. They engaged in nutrition assistance and submitted “activity reports”. Flaws in the open-ended activity report included (1) it took an effort to search for the wanted information, (2) the amount of description and kind of information varied by reporters. Since its format was unsuitable to immediately grasp the activities, we developed a new format (revised first version) that addressed the flaws observed in 481 activity reports and revised (revised second version). These versions were evaluated by a total of 135 participants in the third and fourth “Japan Dietetic Association-Disaster Assistance Team (JDA-DAT) leader training courses” and the latest version was developed based on 124 evaluation sheets. In our latest version, (1) reporters can roughly classify the information while filling up since the latest version specifies what and where they should entry, (2) time for entry as well as analysis of information will be shorten by referring an example of description and abbreviation list of frequently appearing words. The latest version will be used when JDA-DAT is dispatched.
