We report a case of autoimmune pancreatitis (AIP) at the same site as an intraductal papillary mucinous neoplasm (IPMN). A 77-year-old man admitted for evaluation of a pancreatic body tumor was found to have a solid tumor at the same site as an IPMN on abdominal MRI and CT. Fluorine-18-fluorodeoxyglucose and positron emission tomography revealed high accumulation in the mass, with a maximum standardized uptake value of 5.3 in the early phase and 5.2 in the late phase. Based on these results and a high suspicion of pancreatic body cancer, we conducted distal pancreatectomy. Pathological examination showed fibroinflammatory lesions with infiltration of IgG4-positive plasma cells, which is indicative of AIP. In addition, the area of IPMN was disseminated throughout the tumor.
