A 31-year-old woman noticed an abdominal distension, and examination revealed a wandering spleen with an enlarged cyst. It is often necessary to undergo emergency surgery for a wandering spleen, however, since she only presented with abdominal distension, she underwent an elective operation of a partial splenectomy and splenopexy. It was difficult to attach the preserved spleen to the surrounding tissue because the spleen had only had a gastrosplenic ligament. The extraperitoneal pocket was made with retroperitoneum under the diaphgram, we performed splenopexy to replace and fix the spleen in the pocket. She is currently in her follow-up period of over 6 months after surgery without recurrence. There have been many reports on the necessity to perform emergency surgery for a wandering spleen with torsion, however, surgical methods for a wandering spleen with large cysts have been established. We report on elective partial splenectomy in this rare case.
